We are committed to working with those who are most vulnerable and challenging by providing high quality, nurturing and stable homes. We also implement expert therapeutic interventions to support their complex behaviour and needs. Clinical services

At Gradan care we work alongside CCATS who provide an integrated therapeutic package of care. Each package is bespoke to the needs of the young person, the home, their family, and those in the network.

CCATS model of service for residential children’s homes involves their psychological team working in close partnership with our residential staff and external educational professionals to ensure a holistic approach is adopted in the care of each individual child and young person.

Advocacy for young people

At Gradan Care we promote the importance that young people have the opportunities to express their views, wishes and feelings and to also have independent support available when they disagree with a decision or have other complaints. All young people have access to an advocate who visits our homes to raise awareness of their services. They give advice and guidance on a range of subjects and help young people present their views where they have specific issues or concerns.

Bespoke care

Each young person receives individual home-based tuition until a time when they are ready to enter a more formal mainstream school environment. We provide homes from solo occupancy, dual and homes to accommodate up to three young people at any one time. This is helpful to those who may be easily affected by peer pressure or those who find a large communal environment intimidating. Our resilience-led approach offers young people the opportunity to reflect on their past behaviours and to build positively for their future.

Placement stability

In our planned homes, the children and young people we aim to offer care and support to, often come to us following multiple placement breakdowns, each reinforcing feelings of rejection and deepening their therapeutic needs. We understand that placement stability is crucial in providing a secure foundation where a young person or child can feel safe enough to address and explore their inner thoughts, feelings and behaviours. We at Gradan Care, will adopt a Principle of Non-Exclusion, meaning our approach begins with thoroughly planning each young person’s admission. We anticipate and prepare for challenging behaviours, maintain flexible resources to adapt as needs evolve, and remain open and creative in finding solutions to support each young person and child’s success.

CARE SERVICES

Our Homes

Our children’s homes are designed to offer a variety of detached houses located in both semi-rural and community locations, providing accommodation for up to three young people at any one time. Each young person has their own private bedroom, and will be given the choice to personalise, along with separate bathroom facilities, ensuring comfort and privacy.

Every young person will be allocated a dedicated key worker who will meet with them regularly to discuss key topics that are important to the individual. The key worker will also collaborate closely with the young person’s clinician. The clinician will offer therapeutic support, developing, monitoring, and reviewing behavior management strategies tailored to the young person or child’s needs.

We encourage young people to take an active role in the daily running of the home and to participate in making their own decisions, empowering them to take ownership of their lives and choices.
